What does a separate cleaning fee usually cover?

If a cleaning is recommended between routine checkups, how can I tell what the quoted fee includes? Is it for a routine cleaning or periodontal maintenance, and are the exam, X-rays, polishing, or other services billed separately?

What findings might lead a dentist to recommend a different type of cleaning? I’d like to understand what care is being proposed without assuming every office handles it the same way.
