Custom TMJ guard or boil-and-bite first?

I’m trying to decide whether a professionally fitted night guard is worth the extra cost compared with a boil-and-bite mouthguard for suspected nighttime clenching.
My main clue is waking with jaw soreness and tight chewing muscles, although I’m not sure whether I’m grinding. I’d like something that stays in place, doesn’t feel bulky, and won’t make my bite or joint pain worse. A custom guard may offer better retention and bite adjustment, but the price is higher. With an over-the-counter option, I’d want to know the return policy and whether it can be trimmed or replaced if it doesn’t fit.
For anyone who’s compared them:
- Was your pain clearly worse on waking, or did you know you were clenching or grinding?
- Did the custom guard’s comfort and adjustment coverage justify the cost?
- Did a boil-and-bite guard help, or did it shift, feel bulky, or increase soreness?
- How quickly should someone stop using either type if their bite feels different, pain worsens, or the guard causes pressure?
I’m leaning toward asking a dentist to check my bite and joint first, but I’m wondering whether a carefully fitted over-the-counter guard is a reasonable short trial when cost is a concern.

A boil-and-bite guard is worth treating cautiously for suspected clenching: the molding method alone doesn’t show whether its thickness, material, or bite contacts are suitable for TMJ symptoms. A dentist can assess the bite and joint first and explain whether an over-the-counter option is reasonable as a short trial.
Waking jaw soreness and tight chewing muscles would make me especially cautious about a bulky guard or one that contacts more heavily on one side. Stop using it if it makes the bite feel different, increases joint or muscle pain, creates pressure on a tooth, or won’t stay stable, rather than repeatedly remolding it. If cost is the issue, it may also be worth asking whether the dentist offers a brief assessment or an adjustment policy before deciding on a custom guard.
